All Students are required to have the following forms on file:
1. Health Form (annually). Your child's pediatrician can fax their standard health form/physical to 603-863-5114.
Health Forms are annually required.
2.Immunization record (annually) Your child's pediatrician can fax their standard immunization record to 603-863-5114. Kids World Academy will not permit "Religious Exemptions" regarding vaccines.
3. Student Emergency Information Form
This form is used for an emergency information guide and provides individuals authorization to pick up your child. IF YOUR PICKUP PERSON IS NOT ON THIS LIST WE CANNOT RELEASE YOUR STUDENT.
ALL INDIVIDUALS PICKING UP MUST HAVE A VALID ID FOR STAFF REVIEW!!! NO EXCEPTION!
Please sign and return on your student's first day at Kids World Academy
4. Medication authorization Form: This form must include the following: "Fever reducer" and "as described on label".
